OFFICIAL CONTEST RULES
1. How to Enter the Contest:
a. No purchase necessary to enter.
b. Only one winner per family and/ or household.
c. Prizes are given away through contesting in one of the following ways
1. Through the KQJK website by being the specified emailer when a particular song or artist is played
2. Through the KQJK website by being the winning entrant in a specified amount of words.
3. Entering by mail and sending a letter, through the U.S. Mail, being the designated numbered letter delivered to the KQJK mailing address.
2. Eligibility Restrictions:
a. The contest is open to all KQJK listeners who are 18
years of age or older and who reside in the greater
Sacramento area. Employees of KQJK, its licensee,
corporate parent CBS Radio
affiliated entities, affiliated advertising agencies,
participating sponsors/promotional partners, other
radio station in the Sacramento metropolitan area
and the members of their immediate families are
ineligible to participate or win.
b. Listeners are eligible to win a KQJK contest only
once every thirty days. Only one winner per family
and/ or household is permitted in any contest.
c. Listeners are eligible to win a prize valued at
$600.00 or more only once every six months.
3. Prizes:
a. All prizes or prize certificates may be picked up at
the office of KQJK, 5244 Madison Avenue,
Sacramento, CA 95841. Any prize or prize
certificate not claimed within thirty days of winning
will be forfeited by the winner. In the event that a
prize or prize certificate is mailed to the winner, it
will be with the prior written consent of the winner
and therefore, winner assumes the risk of its arrival.
KQJK is not responsible for the safe arrival of a
prize or prize certificate.
b. Contest prizes are not transferable. Prizes may not
be substituted for or redeemed for cash.
4. Selection of Winners:
a. Decisions of station management with respect to the
contest are final.
5. Conditions:
a. Payment of all federal, state and local taxes are the
sole responsibility of the winner. Winners may be
required to sign an IRS Form 1099 or the equivalent.
Contestants who win prizes valued at $600 or more
during any calendar year may receive an IRS 1099
form from KQJK and such prizes shall be reported to
the Internal Revenue Service for the tax year in
which the prize was awarded.
b. KQJK is not responsible for replacing or reimbursing
winners with any form of compensation for events
that are cancelled. All cancellations are deemed
beyond the control of KQJK and its sponsors. This
includes, but is not limited to event cancellations, trip
schedule charges, changes in travel arrangements,
travel delays of any form and duration, and delays as
well as acts of nature. KQJK and its sponsors are not
responsible for any expenses incurred by contest
winners as a result of such cancellations.
c. In the event of any dispute regarding the contest
eligibility, or the contest rules, the decision of KQJK
Management shall be final.
d. By participating in the contest, the winner or winners
agree to have their name, voice, or likeness used in any
advertising or broadcasting material relating to this
contest without additional financial or other
compensation, and to sign a publicity release prior to
acceptance of the prize.
e. Contest winners will be required to sign a liability
release prior to acceptance of any prize or which
prize includes the provision of transportation, whether
or not such transportation is provided by KQJK.
Pursuant to the liability release, the winners will agree
to hold KQJK, its corporate licensee, parent
corporation Infinity Broadcasting Company, affiliated
companies and the officers, shareholders, directors,
employees, agents and representatives of each of them
harmless against any and all claims including the
substitution of a prize of equivalent value or liability
arising directly or indirectly from the prize or contest.
6. KQJK reserves the right to make changes in the rules of
the contest, which will become effective upon
announcement.
7. No purchase is necessary. The contest is void where
prohibited.
8. Failure to comply with the contest rules may result in a
contestant’s disqualification.
9. Copies of the written contest rules are available during
regular business hours at the main studio of KQJK,
5244 Madison Avenue, Sacramento, CA 95841.
10. Odds of winning depend on number of entries
received.

1. How to Enter the Contest
a. KQJK and AT&T’s “Black Crowes Street Date” contest (the “Contest”) will begin on or around 12:00 pm Pacific Time (“PT”) on 3/3/2008 and will end at 12:00 pm PT on 3/16/2008.
b. To participate in the Contest, you may enter via the following method(s):
Listen to KQJK (the “Station”) between 6 am and 6 pm PT, Monday through Friday, from 3/3/2008 through 3/16/2008, for a Black Crowes song. Upon hearing a Black Crowes song, go to the Station website at www.jackdotcom.com, click on the link for this contest, and follow the instructions to enter and submit your first name and last name, complete address, including city, state, zip code, telephone number, date of birth, and a valid email address in the online entry form. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider or other organization that is responsible by assigning email addresses or the domain associated with the submitted email address. Multiple participants are not permitted to share the same email address. All entries submitted become the sole property of the Station and will not be acknowledged or returned. For purposes of these Official Rules, “receipt” of an online entry will be solely determined when the www.jackdotcom.com web server records the entry information. Any automated computer receipt (such as one confirming delivery of email) does not constitute actual receipt of an entry form for purposes of these Official Rules. The Station’s computer is the official time keeping device for this contest. Use of any device to automate entry is prohibited. The Station is not responsible for entries not received due to difficulty accessing the internet, service outage or delays, computer difficulties or other technological glitches. A Black Crowes song will play approximately 2 times per day for the two weeks of the Contest.
c. Only one internet entry per person and one internet entry per email address is permitted for each Black Crowes song that is played
2. Eligibility Restrictions
a. The Contest is open to all persons who are 18 years of age or older and who reside in the Sacramento Total Survey Area, as defined by Arbitron. Employees of AT&T Operations, Inc., CBS Radio Inc., the Station, its licensee, and each of their corporate parents, subsidiaries, and affiliates, their advertising agencies, participating sponsors/promotional partners, other radio stations in the Sacramento metropolitan area and the members of their immediate families and or households are ineligible to participate or win. (AT&T Operations, Inc., CBS Radio, and the Station shall be collectively referred to as the “Contest Entities”) Subject to all applicable federal, state and local laws and regulations.
b. Only one winner per household is permitted in any Contest administered by the Station.
c. Entrants are required to provide truthful information and the Station will reject and delete any entry that it discovers to be false or fraudulent. The Station will disqualify any entry from individuals who do not meet the eligibility requirements, and the Station will also delete any entry received from persons under the age of 13 in compliance with the Children’s Online Privacy Protection Act.
3. Prizes:
a. One (1) Grand Prize winner will receive: A trip for two (2) (winner and guest, age 18 or older) to see the The Black Crowes perform in Atlanta, GA in 2008. [Exact location and date is TBD.] Trip includes round trip coach air transportation from major U.S. gateway airport nearest winner's residence (as determined by Sponsor) to Atlanta, GA, 2-night standard double occupancy hotel accommodations (one room) (as determined by Sponsor), two (2) tickets to attend the concert (as determined by Sponsor, and two (2) passes to a meet and greet with The Black Crowes. Approximate Retail Value ("ARV"): $2,000. Value of trip may vary depending on location of winner's residence. Certain travel restrictions may apply. If winner resides within a 100-mile radius of Atlanta, GA, ground transportation may be provided in lieu of air transportation and no compensation or substitution will be provided for difference in prize value. Travel arrangements must be made through Administrator’s agent on a carrier of Administrator’s choice. No prize substitutions, cash equivalent, or transfer of prizes permitted except at the sole discretion of the Contest Entities. Prize subject to availability and the Contest Entities reserve the right, at their sole discretion, to award a prize of greater or equal value if the advertised prize is unavailable. If winner cannot travel on dates specified by Contest Entities, winner may be disqualified and an alternate winner selected. All other costs not specifically stated herein, including, but not limited to, taxes, meals, tips and ground transportation, are the responsibility of the winner. Taxes are solely the responsibility of the winner. Winner and travel companion must travel on the same itinerary and are responsible for obtaining all required travel documents. In addition, travel companion of Winner must be 18 years of age or older. Winner may be required to present a valid credit card upon hotel check in to cover any incidental expenses incurred during their stay. Winner and travel companion are responsible for obtaining travel insurance (and all other forms of insurance) where applicable, at their option, and hereby acknowledge that the Contest Entities have not and will not obtain or provide travel insurance or any other form of insurance. Winner and travel companion must sign a Publicity, Liability Waiver and Travel Release form prior to traveling. If a selected event is unable to take place as scheduled, for reasons such as cancellation, preemption, postponement or unavailability, or for any reason beyond the control of the Contest Entities, the Contest Entities’ sole responsibility to a Winner will be to award the remaining available portion of the prize and no substitution will be provided for unawarded portion of prize. Tickets are subject to terms and conditions as specified by issuer. Seat locations to be determined by Contest Entities. Contest Entities reserve the right remove or to deny entry to winner and/or guest if he/she engages in a disruptive manner, or with intent to annoy, abuse, threaten or harass any other person at the concert or meet and greet.
b. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The Station is not responsible for the safe arrival of a prize or prize certificate.
4. Selection of Winners
a. Decisions of Contest Entities management with respect to the Contest are final.
b. The ninth (9th) entry received by the www.jackdotcom.com web server each time a Black Crowes song plays will be entered into the grand prize drawing, upon confirmation of eligibility. It is up to the Station’s sole discretion to determine all 9th entries, as received by the www.jackdotcome.com web server. On March 16, 2008, at approximately 12 pm PT, the grand prize winner will be selected in a random drawing of all such entries.
c. Odds of being entered into the grand prize drawing pool depend on the number and order of entries received after a Black Crowes song plays. Odds of an entry from the grand prize drawing pool being selected for the grand prize are 1 in 20
d. Grand prize winner need not listen or be present to win.
e. Grand prize winner will be notified by email and telephone following the grand prize drawing. Winner must execute and return any required affidavit of eligibility and/or liability/publicity release within ten (10) days of notification attempt or prize will be forfeited and an alternate winner may be chosen by random drawing of the remaining entries in the grand prize entry pool. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, potential winner forfeits prize.
5. Conditions
a. Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners may be required to sign an IRS Form W-9 or the equivalent.
b. By participating in the sweepstakes, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this sweepstakes without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, the Station in its sole discretion may require sweepstakes winners (and their travel companions, if any) to sign a liability release, agreeing to hold AT&T Operations, Inc., CBS Radio, the Station, its corporate licensee, and each of their parent, subsidiary and affiliated corporations, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.
d. The Station, in is sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the Station’s website or is otherwise in violation of the rules. It further reserves the right to cancel, terminate or modify the sweepstakes if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
e. The Contest Entities reserve the right to make changes in the rules of the Contest, including the substitution of a prize or equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event is delayed, rescheduled, postponed, cancelled, or has a change of venue, the Station reserves the right, but not the obligation, to cancel or modify the sweepstakes and shall not be required to award a substitute prize.
f. Failure to comply with the Contest rules may result in a contestant’s disqualification solely at the discretion of the Contest Entities.
g. The Contest Entities are not responsible for typographical or other errors in the printing, the offering or the administration of the Contest, or in the announcement of a prize.
